EMPLOYEE WAS DOING MAINTENANCE ON THE CRUSHER SHAKER. WHEN THEY WENT TO REPLACE THE GUARD, IT SLID DOWN CRUSHING THE MIDDLE AND RING FINGERS OF HIS LEFT HAND.
