On 1/26/25, 3 employees were at the worksite to install a conveyor belt. The conveyor belt was lifted from the back end to be suspended approx. 20 ft in the air and the front was on the ground. EE walked up the conveyor belt while it was suspended to take some measurements. EE lost EE's balance and fell around approx. 10:00 am.

On 1/26/25, EE & 2 co-workers were at the worksite to install a conveyor belt. The conveyor belt was lifted from the back end to be suspended aprox. 20 ft in the air & the front was on the ground. EE walked up the conveyor belt while it was suspended to take some measurements. EE lost balance & fell around aprox. 10:00 AM.
