Employee was inspecting the Cone. As he was climbing down the ladder, he fell over head first into the adjacent conveyor belt. He had gotten dizzy other time(s) that day. Initial diagnosis was that he had had a stroke which may have contributed to the dizziness.

Edgewood Aggregates LLC · Caught in, under or between a moving and a stationary object

The injured EE was freeing some rocks stuck in crusher jaw with a digging bar. When they were freed the rocks fell downward, striking the digging bar and catching the tip of the left pinkie finger between the digging bar the the jaw safety platform. The EE fractured and lacerated the tip of his finger.

EE was at the bottom of the open pit quarry. He climbed up ladder of cat 980f loader, to speak with the operator, other ee. Upon climbing down, he stated he stepped on a loose rock, lost his balance and fell down. When he got up he complained og pain in his right ankle. x-rays-sprain of calcaneofilular and fracture ankle 4-6 weeks pending on light duty.
