USING A PIPE WITH RACHET TO TIGHTEN BOLTS,FELL AND STRUCK HIMSELF IN HEAD WITH PIPE.I.NAME WAS WEARING HARD HAT BUT IT FELL OFF JUST PRIOR TO INCIDENT.I.NAME WAS OUT 2 DAYS FOR HEAD INJURY AND 5 DAYS FOR TONSILITIS.

I.NAME HELPING LIFT CONVEYOR BELT.WRAPPED CHAIN AROUND BELT AND A CONCRETE BLOCK USED AS A WEIGHT AND SET IT ON THE GROUND.I.NAME THOUGHT BLOCK WAS ON GROUND AS THERE WAS SLACK IN CHAIN.REMOVI NG CHAIN HE PULLED ON IT CAUSING BLOCK TO DROP 4" ONTO FOOT.BLOCK WEIGHED 3500 LBS.STEEL TOES KEPT FOOT FROM BEING SEVERED.
