Miner placed hand on top of bolt while running the impact wrench causing a laceration to right palm.

Flash burns (electric)

North Pit crushing plant screen 1 feed belt breaker tripped during regular operations. Plant operator (injured miner) seeing no other malfunctions, opened the electrical cabinet to reset the breaker and re-engage the starter motor, which caused an electrical flash arc, causing burns to the operators right arm. after investigation, it was determined proper LOTO was not performed.

Miner 1 was holding a roller frame during the installation process. Miner 2 was driving the frame in place with a 3 lb. hammer. Miner 2 struck the index finger of Miner 1, causing a small fracture on the tip of the index finger of Miner 1.

Struck against a moving object

The injured miner was using a Caterpillar D-10T2 bulldozer to push material parallel to a bermed bench. While traveling in reverse the dozer drove off the downhill side of the bench and rolled over, ending up on its tracks. The total damage to the D-10 is unknown, but the machine appears to be structurally sound and repairable.

Struck by flying object

The Miner was using a handheld grinder on steel without safety glasses and a face shield. A steel shaving contacted EE's right eye.

Struck against a moving object

The miner claimed that the air ride in the seat of the 824 John Deere loader was not functioning and the ride control was also not functioning causing lower back to ache.

Miner slipped while descending a piece of crushing equipment and caught self before falling. Employee felt a pain in employee's stomach two days later and reported the incident. Employee has been diagnosed with an abdominal hernia.

EE was on a screen catwalk, shoveling spilled material off the catwalk from a kneeling position. EE set EE's left hand on the material pile to re position self. Another Miner, who was helping loosen the material pile with a 6' digging bar, struck EE in the hand with the digging bar.

The Miner was standing over a 3"" discharge waterline from a diesel powered pump. The discharge waterline had a small leak in it. When the line became pressurized, it tore striking the Miner in the left knee.

Fall from machine

The miner was helping change screens on a screen plant. He was standing on the discharge belt and working outside of the catwalk guardrail. He stepped backwards off the discharge belt and fell approximately 3' onto a shuttle belt.

Fall to the walkway or working surface

The miner was repairing a torn belt. Due to freezing rain the entire site was coated with ice. The miner slipped and fell. When he fell he twisted his knee. He is required to wear a knee brace and attend three physical therapy sessions.

Fall down stairs

A wood stair used to access the scale house gave way causing worker to fall approximately 2 feet.

Over-exertion in lifting objects

The Miner was helping clean up a steel stock pile. He lifted a piece of steel and twisted while lifting. He felt a sharp pain in his lower back and right leg. He reported the incident to his Supervisor but refused medical treatment. He informed management of the incident on 12/13/2012. He received an MRI on 1/10/2013 and surgery for a herniated disc was performed on 2/14/2013.

Struck by flying object

Worker was cutting a steel chute on a crushing plant with an oxygen/acetylene torch. He was not wearing any eye or face protection. A particle struck him in the left eye. The particle was removed by a doctor with a cotton swab.

Caught in, under or between a moving and a stationary object

Worker was trying to remove the bit from an Ingersoll-Rand 3"" drill with a 16"" pipe wrench. He cycled the drill and with the bit spinning freely in the jaws of the pipe wrench he grasped the wrench. When he grasped the wrench the jaws engaged on the bit pinning the ring finger of his right hand between the wrench and the dust guard of the drill.

Over-exertion in lifting objects

reoccurance of previous injury(per call-EE was moving material on 8/28/09-strained back with no treatment, no lost time no light duty-not reported 9/18/09-complained went to dr-dr gave pain med with physical therapy.

Fall onto or against objects

worker was walking down an incline when he tripped on a rock. He put out his right hand to break his fall striking it on another rock. He suffered a laceration on the palm of his right hand that required four sutures. Fatigue and an uneven surface contributed to the accident.

Worker was operating an articulating Volvo rock truck down an incline and came in contact with an uneven section of road, causing the bed to overturn.

Worker was walking down a slope to restart a stacker belt and stepped awkwardly, re-injuring his knee that he had had surgery on 7 years prior.

Struck against a moving object

Worker was backing up a bulldozer. The machine rocked backwards and the worker's body motion went foreward causing a sharp pain in his lower back.

Caught in, under or between a moving and a stationary object

Worker was manually rotating the electric motor to #19 jaw in search of part numbers. Due to the counter balance of the jaw, the electric motor reversed rotation and the worker's thumb was caught in between the fan and the motor housing.

Struck by falling object

A cheat plate was being installed in the Telsmith jaw. The worker had taken off his hard hat to check the bolt hole alignment. One of the loose bolts used for the cheat plate was located on the upper part of the jaw and rolled off, striking the worker on the head.

Two workers were re-installing a pulley guard on a jaw. One worker lost his grip on a 2 lb hammer. When the hammer slipped from his grip, it struck the other worker on the chin.

Struck against stationary object

Worker was assisting Maintenance worker repair crusher. He moved in an upward motion, striking his hardhat on machinery. Worker complained of neck and shoulder pain the following day.

Working alone changing cutting edges on a Kowasaki loader, taking old bolts out, a bolt had a sharp edge. It spun around cutting his finger on the left hand.

Worker was broadcasting sand on a conveyor belt that had ice buildup and was slipping. He extended the shovel too close to the moving conveyor. When the shovel made contact with the belt it was forced towards the worker causing a shoulder strain. Cleared to return to work the next day.

EE WAS CLIMBING UP FRONTEND LOADER STEPS AND FOOT SLIPPED. HIS RIGHT ARM(HAND) LOST GRIP AND HE SWUNG BACKWARDS, HOLDING ON WITH HIS LEFT ARM(HAND). CO-WORKER PUSHED HIM BACK TO LADDER BUT HE HEARD A LOUD POP IN HIS SHOULDER.
