Employee was exiting Cat D-9 dozer and slipped from the last step. Employee strained knee when employee landed. This didn't become a reportable incident until 1/22/24.

D-9 dozer was pushing material when the operator turned the machine and pushed off the side of the cut. The machine went too close to the edge and the machine rolled off the embankment. It turned over completely to its right side and down a 20' embankment landing on its tracks. The operator was unharmed except a couple of bumps.

Lube tech was greasing a 966 front end loader when the grease hose ruptured resulting in a pinhole puncture in the employee's RT index finger and possible injection of an unknown amount of grease into the finger. Was sent to ER for evaluation and treatment to ensure no serious damage and to treat for infection.

EE was replacing a bearing on a track jaw crusher tail pulley. EE was holding a bar while EE's supervisor struck it with a sledge hammer to push the bearing down the shaft. Sledge hit EE on RT index finger causing laceration and fracture of the tip of the finger. Treated with stitches and splint at nearby Workmed office.

Employee was doing repairs to a drive belt motor. A bolt was removed that allowed the motor to pivot and swing from its position. When the motor swung, it hit employee's hand and smashed ee finger between the motor and the housing. EE was wearing gloves but received a laceration.

EE was helping the crusher crew lower the stacker belt of our portable crusher. The hydraulics were turned on to lower the stacker belt and the EE rested his hand in an area of the stacker belt that he shouldn't have put his hand. A piece of metal on the stacker belt came down on his right middle & pointer finger, which smashed, lacerated and fractured parts of his fingers.

EE WAS SHOVELING DIRT & ROCKS FROM UNDER A CONVEYOR BELT. WHEN HE WENT TO STAND UP STRAIGHT, HE COULDN'T AND HE FELT AN INSTANT SHARP PAIN IN HIS BACK WHEN HE TRIED TO STAND UP. CASE INVOLVED MODIFIED WORK.
