Absorption of radiations, caustics, toxic and noxious substances

Employee was cleaning foam spray out of a hose at the end of a shift. Some of the remnants of the foam got into the employee's eye.

EMPLOYEE WAS LIFTING 50LB BAGS OF ANFO, TO LOAD A ROUND UNDERGROUND. COMPLAINED OF ABDOMINAL PAIN ON 06-28. WENT TO THE DOCTOR'S AND WAS DIAGNOSED WITH A HERNIA RESULTING FROM STRAINING OR LIFTING HEAVY IEMS. PUT ON LIGHT DUTY. WAITING ON DETERMINATION IF THIS IS A WORK RELATED INJURY.

Struck by flying object

Employee was reaching down to light an empty bin. As ee did this ee rested left hand on the forklift mast, exposing left hand thumb. As the employee stood up the bin came up quickly hitting left hand thumb.

Employee was resetting a damaged post. Employee's partner noticed dribbling from the ceiling and yelled at employee to get out. Employee got up and started moving towards the opening. Slab fell from the ceiling and caught the employee underneath.

EMPLOYEE WAS PUTTING LAGGING OVERHEAD WITH THEIR PARTNER. IT WAS A 3X12X6 TIMBER. THE EMPLOYEE LOST HOLD OF THEIR END AND IT HIT THEM ON TOP OF THEIR HARD HAT.

EMPLOYEE WAS OPERATING THE SKID STEER AND WAS MIXING MATERIAL IN THE BAY AREA OF THE ALPINE MILL. THE BUCKET ON THE SKID STEER WAS FULL OF MATERIAL AND TIPPED FORWARD CAUSING THE EMPLOYEE TO HIT EMPLOYEE'S HEAD ON THE INTERIOR OF THE SKID STEER. EMPLOYEE HAD A CONCUSSION AND CUT ON EMPLOYEE'S NOSE.

EMPLOYEE WAS TYING IN A ROUND OF DYNAMITE. WAS LOOKING UP AND SMALL ROCKS FELL FROM THE CEILING INTO HIS RT. EYE. THE ROCKS HAD PRELL ON THEM. THE EMPLOYEE HAD SMALL DEBRIS IN HIS EYE AND A SCRATCH, BURN ON THE EYEBALL. EXPERIENCED BLURRY VISION OUT OF THE RIGHT EYE.

EMPLOYEE WAS CHECKING GROUND CONDITIONS AND HAD HIS BACK TO A WALL WHEN A ROCKSLAB APPROXIMATELY 20 PDS PEELED OFF THE WALL AND PUSHED HIM TO THE GROUND. EMPLOYEE TRIED TO STEP AWAY BUT THE SLAB CAUGHT HIM ON HIS RIGHT SIDE CAUSING HIM TO FALL TO THE GROUND.

While working to correct faults in electronics of a Sullivan air compressor something caused the electric control panel to explode causing severe burns on the hands and face.

We were pulling out tube and started laying it onto tube rack when the over shot popped off the retriever point causing tube to fall. Other end of tube came up and hit employee on side of cheek. He then fell to the ground. He was unconscious for a few seconds.

Miner was installing 5' split set rockbolts. He had pushed the bolt up into the back hole and went to push the bolt into the hole with the jackleg drill, when the bolt with the plate and mat fell down, hitting the miner in the mouth.
