Struck by falling object

Subject was changing a 2 inch pipe on the fuel truck. The pipe slipped off the truck and hit the subject on the right side of the face.

Subject had just came out of the bath house. Subject stepped on a loose rock and rolled an ankle.

Subject was turning on a generator and slipped on some ice.

Miner was standing on the tracks of a D8 dozer. EE had pressure washed the dozer and the tracks were slick. The miner slipped and twisted EE's left knee.

Struck by flying object

Operator was pushing overburden with a D11 dozer #108. A rock hit the left side window and shattered the window. The glass cut the operators left elbow.

Subject was trying to get some batteries out of a battery box on a service truck. The subject was using a pry bar for leverage. The pry bar slipped and hit the subject in the upper lip on the right side.

Struck by flying object

Subject was changing teeth on a trac hoe bucket. The pin holding the tooth was packed with dried mud. The subject put a socket on the pin and hit it with a sledge hammer. On the second try the socket, after being hit with the sledge hammer flew up and hit the subject in the face and eye. Safety glasses protected any injury to the eye.

Subject was changing the cutting edge on the Bobcat skid steer. The subject was tightening the bolts. The bolt head spun when it was being tightened and cut the subjects hand.

Subject was on a drop deck trailer unhooking cables from a transformer that had been loaded. Subject went to dismount the trailer and slipped and fell on EE's right leg.

Mechanic installing a grill on a Cat D11 dozer. The subject slipped and fell on EE's left arm.

Co-worker was brushing glass off clothes when the subject got some glass in an eye.

Miner had moved the stacker conveyor on the auger miner. Miner got out of the cab and the door came shut and hit the miners middle finger.

Miner was working on a crusher in the coal yard. Miner came to the mine office and cleaned up. During the process of washing EE's face something got in EE's right eye

Miner was moving 6 inch discharge hose. PC650 trac hoe operator was helping to move hose. The operator lowered the bucket of the trac hoe on the miners left foot.

Miner was climbing the steps on the water wagon to start the rear motor. Miner slipped because of muddy boots and fell off the steps hitting their shoulder on the ground.

Miner put a new tire on a rim. Miner was filling up tire with air. Miner took air hose off the valve stem and tire blew. Tire struck miner on arm and face.

Highwall mining in Pit #9 in hole 8 panel 2. The cutter head became lodged around 400 feet. Working with MSHA a recovery plan was accepted and the lodged cutter head was removed.

While mining with Cat HW300 highwall miner we started having communication problems with the cutter head. While extracting the cutter head it became lodged in the hole. A recovery plan has been accepted to retrieve the miner.

There was a slough that occurred in Pit#24 about 250 feet east from the active pit. A Komatsu 490 track hoe was sloping the area when the slough occurred and partially buried the track hoe. Under the direction of MSHA a plan was implemented and the track hoe was removed with little exposure to any miner.

On 1/15/14 an inundation of mud occurred in Pit 27 on E highwall. This caused material to be deposited on Komatsu 2000 excavator to railings of opr's deck, around R side of excav. & in front of opr's compartment. Cat 992 loader on S side of slide had mud deposited on L side of wheels. No inj. as a result of slide. Pursuant to cit. issued # 8482945 this report is being filed.

Subject was cleaning out the crusher because it was plugged. The subject was using a bar and was chipping away at the coal. The bar slipped and the subject hit the metal housing of the crusher with his right hand.

A dead rattlesnake was placed on the 980 loader platform, which startled the miner who then jumped/ slipped from the ladder causing said miner to twist his knee.

Struck by flying object

Subject was helping change a corner cutting blade on a D10 dozer. When the bolts were removed from the plate, the plate fell hitting a pair of 18 inch channel lock pliers. The pliers flew up and hit the subject just above the right eye.
