Fall to the walkway or working surface

The employee dismounted a fork lift and walked around it and slipped on some ice and landed on left forearm causing the injury.

Struck against stationary object

The employee dismounted the fuel truck. EE hit EE's hand on the body of the truck. Doctor's findings EE had an old fracture volar plate fracture on the fourth right middle phalangeal base. EE received medical attention and has returned to work.

Yard hand and truck driver were securing load on flat bed truck. They were using chain and chain binders, when cheater pipe slipped off of lever chain binder. The cheater pipe hit yard hand in the left eyebrow.

Employee was busy walking to the Har-El drift entrance. He slipped on some lose gravel and lost his footing. He fell on his upper arm.

Struck by falling object

Bolting with a Jack leg and rock drill installing split sets and Hog wire. The operator was drilling a new hole in the back. He penetrated the steel about eight inches when the steel snapped on the shank. The remaining piece of drill steel dislodged from the drill hole falling three feet striking ee on his upper left thigh

Caught in, under or between a moving and a stationary object

Employee loaded a pump into an L.H.D bucket. He moved away and signaled the operate to curl the bucket. The pump started to fall out employee jumped into the bucket and pushed the pump into position. Whilst he was busy with this action the bucket crushing his left middle finger tip against the stopper.

Employee was stepping down to ground level from a stationary piece of equipment in the mill. Slipped on loose ground and braced his fall with his left wrist resulting in a fracture. No equipment was damaged and employee sought medical attention the next day.

EE appeared to go into shock after being questioned about a specific task he had preformed. He exhibited seizure like symptoms.

Employee exited PC-200 excavator, surface mine excavations machine, and was walking on level surface ground when he slipped and fell, stumbled on loose rock. Employee struck wrist on ground and fractured wrist.
