The rig welder was cutting on a 2" pipe with a 7" metabo disc. The welder was in the process of cutting out a weld tack on the bottom side of the pipe. The welder was on EE's knees when the grinder kick back at EE, cutting EE's left cheek. The welder was using EE's welding hood for protection from the sparks and debris.

The injured employee was working from a height of about 14 feet. What led to this event was that another employee was removing a drive pulley off the crusher. Injured employee took it upon self to help without donning any fall protection.
