Miner was loading trash pallets into trailer, one fell back and hit left wrist and then pulled on arm straining left shouder

Mined was operating street sweeper and jarred lower back

Lifting a tote of gravel into shaker strained shoulder

Operator was exiting trackhoe and stepped onto unstable footing and fell down twisting ankle and cutting hand on a rock

Miner was cutting a bearing off of a conveyor with a torch, hit part with a hammer and a small piece of metal flew off and stuck in their lower lip, Dr removed metal and put 2 stiches in lip

Installing Conveyor support beam, chain slipped on support and beam pivoted knocking miner from the catwalk, fell approx 8 feet, slight fracture of wrist

Slipped climbimg into machine and fell backwards onto left wrist

Miner said someone was welding and a spark went down their boot and burned their foot, Miner later said they were cutting a self tapping screw with a grinder and a piece went down a boot and burned their foot. When asked to come to office and write a report of what happened, where it happened, they did not want to make a report.
