Miner was cutting a bearing off of a conveyor with a torch, hit part with a hammer and a small piece of metal flew off and stuck in their lower lip, Dr removed metal and put 2 stiches in lip

Fall from scaffolds, walkways, platforms

Installing Conveyor support beam, chain slipped on support and beam pivoted knocking miner from the catwalk, fell approx 8 feet, slight fracture of wrist

Slipped climbimg into machine and fell backwards onto left wrist

Miner said someone was welding and a spark went down their boot and burned their foot, Miner later said they were cutting a self tapping screw with a grinder and a piece went down a boot and burned their foot. When asked to come to office and write a report of what happened, where it happened, they did not want to make a report.
