Over-exertion (Not Elsewhere Classified)

Employee was drilling a hole in angel iron metal for door frame. Employee was not using the secondary handle during drilling. The drill spun around, twisting employee's right hand and striking EE on back of hand.

Fall to the walkway or working surface

Employee was walking through the drive/walkway between lab building and sample building on the premises. EE stumbled and fell on left wrist due to uneven pavement while walking to collect a sample container.

EE was placing a bulk bag on the unloader. It would not go up high enough to get the discharge spout over the hole to be able to untie the bulk bag. EE had untied the bag and was pushing over in the hole. EE fingers were pushed against the edge of the opening. The edge went between 2 fingers on left hand.

Employee was putting the dock board down and caught his 1st finger (right hand) between the dock board & dock. Employee was wearing gloves.

EE was walking down driveway stepped off path onto grass instead of continuing on driveway, turned left ankle, lost her balance and fell down on right knee.

Employee stated that he bent over to stencil a bag, then felt a pain in his lower back. Employee had trouble straightening up. Employee reported incident at 7:45 a.m. and stated he felt better moving around, then returned to work. Employee was sent to hospital for evaluation at about 9:45 a.m.

Employee was stretch wrapping a pallet of material and did not pay attention to where he had stationed/placed the portable pallet jack. Employee tripped over the pallet.
