Over-exertion (Not Elsewhere Classified)

Employee was restacking a pallet of material by hand. As employee twisted to set bag on pallet EE felt pain to EE's back. Employee was transported to hospital at Radford, VA. MRI indicated a slipped disc in back. Employee was advised to return to work in 7 days.

Employee was holding trash bag open for co-worker to dump dirt/dust from sweeping up. Was not lifting bag or moving it. While bent slightly over, felt a shock like pain in middle/lower back.

Employee walking across floor and heard knee ""pop"". Did not trip or fall. No debris on floor. Did not go to Dr. that day. Worked 12 hour shift next day. Went to DR. on 10/19/20. I was not notified of incident until 10/19. Employee was out of work 7 days and returned to full duty on 10/28/20.

Employee getting out of forklift and twisted knee. No fall or slip. Employee continued shift. Went to doctor 6 days later, put on light duty. Tested positive for Covid on 4/28, returned to work 5/11 with no restrictions. Had surgery on knee 7/30/20. Has not returned to work as of this date, 10/26/20.

Employee was opening sliding door entering bagging plant and failed to release handle, mashing middle finger.

Struck by falling object

Employee was cleaning frozen limestone from power screen chute. Chunk of limestone fell from above and hit employee on head and face area causing abrasions to face and head.

Employee was installing V-belts on motor/speed reducer. Unknown to employee, motor mount securing motor to speed reducer had broken & during belt change, motor fell off base & fell in his lap. Motor could not be moved by employee & his helper. Help was summoned & motor was lifted off employee. He had bruising to right leg & wrist, but returned to work after being examined at ER.

Employee walking down steps in Mill and stumbled on step, falling on right elbow.

Operator stepped between rollers on conveyor and caught his foot, resulting in twisted ankle.

Employee was working on the Pellet Plant dryer when he tripped and fell, cutting his right forearm on a piece of protruding metal. He was taken to Ft. Chiswell Medical Center for treatment, requiring 8 stitches. Afterwards, the employee returned to work for the remainder of the day.

Fall from machine

Customer driver was tarping trailer when he lost his footing and fell backwards approx 6 ft to the ground. The skin was broken on his scalp & left arm was tender & swollen. !st Aid was administered & he was transported by EMS to hospital. HE was treated & released with instructions to have follow-up x-rays to determine if elbow is broken. He is still under doctor care as of 6/6/08.

Placing pump on top of channel pump fell pinning his pinky finger on right hand between pump and channel.
