THE SECTION FOREMAN WAS PULLING CABLE AWAY FROM MINER AS THE MINER WAS BEING MOVED TO 5 PILLAR. THE CABLE BECAME TWISTED AROUND MINER OPERATORS RIGHT ANKLE AND TWISTED HIS KNEE.

THIS ROCK FALL WAS FOUND BY KEN MULLINS AT APPROX. 400 AM ON 5-7-96 LOCATION OF THE FALL IS IN THE CROSS CUT BETWEEN THE NO.3 AND NO.4 ENTRY SPAD MOS. 983 AND 876. SIZE OF FALL IS 16' WIDE 28' LONG 0-5 THICK AREA IS BOLTED WITH 3' CONVENTIONAL BOLTS WITH 4' POINT ANCHOR BOLTS INSTALLED IN THE INTERSECTIONS FALL WAS A SLIP THAT WAS UNDETECTED WHEN TEST DRILLED.
