Roof fall 1st crosscut underground in the #2 entry extending 15' towards #1 and 60' inby 6-8' thick. Crushed intake stopping, belt infrastructure and high voltage cable. Roof contains hillseams, soft shell and water. Fall occurred after a hard freeze and thaw.

Guiding scoop to load, bucket pushed concrete block and pinned employee's rt. foot. against belt. Trans. resulting in fracture and laceration. Mud and water on floor.
