EE WAS GREASING THE TAIL PULLEY BEARINGS. WHILE THE PLANT WAS LOCKED OUT TO CHANGE SCREENS. HE WAS STANDING ON TOP OF TWO ECOLOGY BLOCKS WHEN HE LOST HIS FOOTING (SLIPPED) AND FELL TO THE GROUND, LANDING ON HIS SIDE.

THE EMPLOYEE WAS NOT WEARING A SEATBELT, AND STOOD UP IN A BOBCAT 742 FE LOADER TO MOVE SOME LOW HANGING HOSES, WHILE STANDING THE BUCKET BLEED OFF AND PINCHED HIS HEAD IN BETWEEN THE CAB GUAR D AND AN I BEAM ON THE FEED HOPPER.
