Loader operator backed the loader away from the rock pile and hit a granule truck that was behind him. The granule truck was moving as well. There were no injuries and some damage to both vehicles. The Loader operator was taken in for a drug screen and was cleared to go back to work.

Loader operator was standing on a 4-inch ledge, looking into the elevator. His foot slipped off the ledge, and his elbow hit a stack of pallets. He heard a pop and experienced pain in his shoulder.

IKO Pacific, Inc · Struck against a moving object

Non-miner trucker delivering limerock didn't retract tarp on side dump truck. Dumped load, tarp held, causing opposite drive wheels to lift 3 ft off ground. Released tarp from top side of bucket, load released. Truck settled abruptly, throwing driver onto truck frame. Injuries to back of head, lower right arm, back.

EMPLOYEE WAS DRIVING IN A NEW KEY WAY TO HOLD THE BULLRING ON THE RAYMOND MILL. HE WAS SWINGING IN HIS HAMMER THROUGH AN ACCESS DOOR TO THE RAYMOND MILL. ON HIS FOURTH SWING HIS RIGHT INDEX FI NGER WAS SMASHED BETWEEN THE HAMMER HANDLE AND THE FLANGE OF THE DOOR. HE FRACTURED THE TOP OF HIS RIGHT INDEX FINGER.
