Metal bits from vertical shaft impactor had broke from auger & appeared on belt. Employee walked over to survey while visually checking operation, a piece of loose metal bounced off back retaining board & struck EE on his left index finger causing an injury that required medical treatment. No lost time & no physical restrictions.

Operator was attempting to close the front windsheild on excavator when it became stuck. He used both hands to force the window shut. It came down quick and caught his right middle finger between the window and the frame.

WHILE A LOADED TRUCK AND TRAILER APPROACHED THE SCALE THE TRUCK ONLY WAS ON THE SCALE AND WAS SLIGHTLY OVER WEIGHT. THE DRIVER OF THE TRUCK GOT OUT OF THE TRUCK IN AN INACCESSABLE AREA, SLIPPE D ON THE SCALES AND FELL TO THE GROUND. SAFE ACCESS IS AVAILABLE WHEN TRUCK IS PULLED COMPLETELY ON SCALES AND ALLD RIVERS ARE ADVISED TO STAY IN TRUCK UNTIL SUCK TIME.
DIGGING OFF CONVEYOR AND IT STARTED TO ROLL AND HE ROLLED DOWN THE CONVEYOR.
