On April 19th employee stepped down off a rock, his step was approx 1-2 feet down off the rock. He came to work on the 20th but was complaining how his leg was bothering him, he went to the Doctors office on the 20th his doctor said he tore his femoris muscle and did not want him to work the rest of the day.
Employee was adding anti freeze to our loader, he turned to get more anti freeze, he lost his footing and fell towards the step ladder hitting the ladder then the ground, his fall was approx 3 feet. He was asked if medical attention was needed he said no he is fine. 4 days later he wanted to be seen by the doctor.
The employee was helping remove a hydraulic tank from the mounting plate of our drill, the tank slipped out of his hand and landed on his finger tip and got pinched between the plate and tank. The employee received 4 stitches on the tip of his finger. He was released the same day to go back to work. Limitations are as follows ""must accommodate finger splint dressing""

On 11/18/11 EE was asked to remove snow from the loading dock area with a loader, when finished moving the snow EE left the loader to see how many pallets were remaining to be loaded, as he walked back to the loader he slipped and fell on the ground. I asked him if he needed to see a doctor he said he was fine. On 12/27/11 EE wanted to see a doctor about his back pain.
