Employee was placing a pump in cold storage. Pump frame swung around hitting employee's leg.

Struck by falling object

EE was doing repair work in shop. Heavy steel ring from disassembling a cone dropped to floor. Steel ring landed on upper part of right foot. (Above steel toe protection). Possible surgery to relieve blood pressure in right foot.

EE has infection to right knee. Infection due to embedment of welding spatter. Treated at hospital with IV for infection.

Employee was cutting rubber material with a utility knife. Slipped with knife cutting left index finger - required three stitches.

Employee was repairing conveyor drive system. Standing on conveyor system, conveyor belt moved backwards causing employee to lose balance. Employee fell backwards on conveyor and became trapped in hopper at lower end of conveyor. Medical - critical condition - possible internal injuries.

Employee was pulling on a pipe, did a sudden jerk on pipe, strained lower back.

EMPLOYEE WAS UNLOADING I-BEAMS FROM LOADER BUCKET. A BEAM FELL ON HIS LEG RESULTING IN BROKEN BLOOD VESSELS AND SWELLING OF RIGHT LEG.

Ee was changing hoses on scraper equipment. Ee fell to ground (approx 10') - injury was breaking of wrist and mild abrasion to face.
