Trailer gate was chained open to the right side of trailer. Employee was standing behind trailer washing trailer out. Chain holding trailer gate gave way and swung down striking employee's right ankle and foot.
Employee was lifting 50 lb totes off conveyor onto pallets, twisted to set tote on pallet, strain of lower back. Was working with 3 other employees did not mention that ee strained lower back, waited 3 days to come in and file out paperwork of injury.
EE WAS GOING TO A PARTS BUILDING TO RETRIEVE A PART. SAID HE WAS RUNNING AND SLIPPED ON ICY AREA AND FELL AGAINST A BOBCAT BUCKET TWISTING HIS LEG.

ANOTHER EMPLOYEE WAS WELDING ON A PIECE OF EQUIPMENT WHEN INJURED WAS EXPOSED TO THE FLASH FROM WELDER ELECTRODE.
EMPLOYEE WAS INSTALLING A RETURN ROLLER WHEN IT FELL APPROX 3" PINCHING HIS LEFT ARM CAUSING A SMALL PUNCTURE REQUIRING 3 STITCHES.
