Mountain Stone Works LLCMetal/Nonmetal

The miner was doing routine maintenance on the screen plant which required him to be on a ladder to check for excessive heat. While doing this the ladder shifted and the miner fell breaking his left wrist. The miner was three and a half feet up on the ladder when the fall happened. He was transported to a hospital and received medical attention for his injury.

PUNCHING HOLES IN BELTING. HIT THUMB WITH HAMMER BREAKING THUMB.

COMPLAINED OF SORE BACK FROM RIDING IN LOADER. WENT TO CHIROPRACTOR NOON ON 7-9-02. RETURN TO WORK 7 AM 7-10-02. LOST TIME 4 1/2 HRS.

HE WAS GREASING VIB FEEDER AND STEPPED BACK AWAYFROM FEEDER, STEPPED ON SOMETHING AND SPRAINED HIS RIGHT ANKLE. WAS NOT REPORTED UNTIL 6/18 AND THEN SENT TO CLINIC FOR EXAM.
