Employee was splicing wires with a utility knife. Blade became dull and slipped on the wires cutting left hand between thumb and index finger. Wound required three stitches.

Caught in, under or between a moving and a stationary object

Removing hydraulic block to prep the excavator for welding. The hoses were tangled, and during an attempt to untangle, hand got pinched between the main frame and the block. Open comminuted fracture of proximal phalanx of finger.

EE stopped the 500 to fuel up. When EE stood up in the cab, EE pivoted left foot while it was still planted. This caused EE to re-break ankle, which had previously been broken.

EE was pressure washing a crusher and got material in his eyes.

Caught in, under or between a moving and a stationary object

EE locked out and tagged out the impactor and was removing rocks. While removing the rocks he hit his right hand between the hopper and the rock. He did not go to the walk in clinic until Saturday, 7/21/12 to have the minor swelling checked out. There were no fractures in his right hand and received a return to work form with no restrictions.

Contact with hot objects or substances

The employee was lighting an acetelyne torch. The torch ""popped"" as he lit it. The flame went up the employees glove and burned the palm of his left hand. The employee received a 2nd degree burn on the palm of his hand.
