Employee was reaching into a partially opened tin of welding rods and the lid cut their arm above the wrist. Being in a hurry and not completely removing the tin lid caused the accident. Employee received 7 stitches to close the wound.

An employee was operating a Dingo MT-100 to clean out underneath a conveyor structure. While backing out from under the structure, the employee's left elbow struck the x-frame of the structure, resulting in a fracture of employee's wrist.

Two Employees were working on a small receiving hopper attached to a conveyor. When the employees loosened the bolts to rotate the hopper, the hopper fell to ground, striking one of the employees in the shoulder.
While working on a conveyor, the employee was adjusting the backsplash to center load the conveyor. While the employee was adjusting the backplate, the employee's grip loosened causing the plate to slip from a controlled grip and landed on the tips of two fingers, pinched between angle iron of the backplate and frame of conveyor.

On 1/29/22, an employee was pulling apart piping in the dry plant. Sand landed/hit the employee's face while using an impact drill to remove bolts from a flange.

Employee was moving a distribution box that was on a pallet with a pallet jack. While pulling the pallet jack towards self, EE felt a pain in RT forearm. Employee has a pulled muscle in the RT forearm.

An employee was operating a loader removing sand from underneath a tail section of an incline conveyor. The employee's RT hand came in contact with the belt causing employee to lose control of the loader, driving the loader into the conveyor. Employee sustained a laceration and fractured RT index finger.
Employee apparently suffered a lower lumbar strain to the back while shoveling. Employee did not report the injury to mine management until the next morning when EE called in.
EE was helping unload material from a semi-trailer. EE went to pick up some dunnage used as spacers for the materials and it was nailed together so ee pulled up hard and the nails gave way easily and the 2X4 hit ee in the lip. EE cut lip, so ee went to the doctor and received 3 stitches. There was no lost time or restrictions put on ee. EE healed nicely.
Employee was operating a Vermeer S450TX in confined quarters and caught EE's left hand between I-beam and the equipment, thus causing a laceration to the palm of the left hand that required stitches.
Employee was operating a 982 CAT Loader. The employee was traveling down a service road and failed to see that the road had been bermed off due to construction. The employee drove the loader through the berm, which caused the loader to bounce. The employee suffered cuts to the forehead and upper left arm when the side window broke out.
Worker was bending over at the waist tightening bolts on a wash plant suction line connection for a pump. As EE applied pressure to the bolt EE felt a pull and pain in EE's lower back.
Employee was cutting loose a steel plate at the ABC reclaim hopper with a torch. As EE was cutting the plate loose, the plate became unstable and fell on the employee's left leg. The employee was performing maintenance on the hopper at the time of the incident.

The employee was pushing a wheelbarrow and hit a concrete curb with the wheel which caused the wheelbarrow handle to hit their left knee.
Employee was operating the walk behind TX 1000 Dingo in #4 re-claim tunnel. As the employee transitioned the machine from the concrete apron to a downhill sloping sandy surface, they were flipped over the top of the machine landing on their face. They suffered a laceration to their face requiring 16 sutures.
The employee was using a wash hose at the Primary wet screen. Employee was coiling the hose and dropped the nozzle on the concrete causing the water to come on. As employee tried to recover the hose, they twisted awkwardly pulling a muscle in their back.
Employee was using a portable grinder and accidentally had a metal sliver get in eye. The employee was wearing safety glasses and a face shield at the time of the injury. EE had sliver removed at the local hospital.
Employee was working to align flange holes on an 18"" diameter pipe. Employee lost balance while coworker was driving a bull pin with a sledge hammer. Employee reached out to catch self and finger landed on the end of the bull pin right as the other employee was striking it. The weather was reported to be calm winds and approximately 45F.
SLIPPED ON BLACK ICE - FRACTURED RIGHT FIBULA

Employee slipped on some hard mud after exiting the Secondary wet plant screening tower.
The employee was operating a CAT 349F on a bench in the quarry. EE was attempting to cast material down hill in order to make the pad wider so EE could safely scale the high wall. EE traveled the excavator over a narrow section and the material gave way from under the outside tracks causing the excavator to tip on its side and slide down the hill.
Employee went to step on to a bulk truck EE was loading and missed the last step of the safety cage causing EE to lose balance and fall on to EE's left shoulder.
Employee was walking across the employee parking lot to the maintenance truck and slipped on a patch of ice and fell to the ground. A fresh snow fall had covered some icy patches.
