snow and ice conditions on gravel near mine site. Miner was walking and slipped on ice, falling on EE's ankle breaking and dislocating ankle.

Miner was setting up crushing spread when EE's legs got pinched between the conveyor and crushing plant. Received stitches on EE's right leg and swelling. No broken bones.
It was snowing heavily and operator was walking to vehicle when operator slipped on ice and fell on side hurting left side rib area.

Miner was moving Large boulder in bucket and the boulder rolled off the bucket causing the loader to jump from the weight being undistributed. Miner banged forehead and got a small cut on forehead that required 5 stitches.

Employee was doing pre inspection on conveyor and wanted to move a large rock off of conveyor and while moving the rock the rock pinched EE's pinky between the rock and metal part of the conveyor. Had to have surgery to repair pinky.

A leased dump truck was dumping aggregate on the floor of our pit and the truck tipped over on the drivers side as the dump box was lifting. The driver sustained 3 fractured vertebrae and a bulging disc due to the impact of the tip. They were not wearing a seatbelt at the time. The driver was taken by ambulance but later released.
