While getting off plate feeder his foot got caught between plate feeder and side board.

Backing haul truck under loader bucket. The hyd. failed and the bucket came down on bed of truck. Ee twisted his neck.

Taking out the hyd. adjustment on HP700 and it fell on his foot.
