Fall to the walkway or working surface

Shoveling out the gyro when something hit him from behind and knocked him down. It was a large rock spraining his right ankle.

Struck against stationary object

checking screens on a shaker when he grabbed the top of shaker wall and cut his hand

Over-exertion in pulling or pushing objects

Turned the wrong way while moving a lifting cable out of the way. Didn't go to the dr. till 10/04/06.

Over-exertion in lifting objects

Shoveling rock that had fallen from a belt, hurt his right wrist. Didn't go to the doctor till 9/13/06.

Struck by falling object

Unbridging SB2 bin when mud clot came down and bent left hand back hurting his wrist.

Fall from machine

He stepped off of the fender of the volvo, lost his balance and fell.

Pulling the counter weight off the right hand shaker and the counter weight fell on his right hand

unplugging chute that had a piece of metal in it when the metal cut his right index finger.

Lifting the cap of the HP 500 pinched his right little finger between the cap and the crusher.

Over-exertion in pulling or pushing objects

was pulling on fuel hose of a 992 loader when felt pain in his arm and elbow. Thought it would get better so waited to go to the doctor. It continue to hurt so when to the doctor on 3-7-06

Flash burns (welding)

Welding in the gyro and the flash from another welder flash burned his eyes.

Struck by flying object

While watching hub hammer remove concaves from gyro crusher, was struck with flying debris. Had laceration to the chin.

lifting screen when slipped out of his hand cutting the top of his hand
