Fall to the walkway or working surface

EE was walking on a catwalk and carry come-a-long and right foot slipped and ee slid on catwalk landing on left knee.

Employee was straightening up a clamp bar on a conveyor belt, using a hammer to line it up and hit thumb.

EE was working on a screw that was stuck. EE was turning it backward to get it unstuck and that's when the v-belt smashed left index finger

Struck against stationary object

EE was holding metal piece while tightening bolt with other hand. When the bolt was tight ee moved left out of the way and finger was cut on a burr from the metal ee was holding. Left index finger

Fall down stairs

Employee was walking down the stairs of C6 conveyor when one (3rd step) of the stairs gave away and employee fell through the stair well.

EE was removing a guard on the counter weight of C4X when the guard pinched right hand middle finger.

Load of angle iron was being dropped in a new location when the load slipped and fell. One piece of angle iron bounced and hit EE in the lower leg. EE had a laceration with stitches.
