Fall from machine

Finished on bridging feeder; when he was coming down the hand rail broke causing him to fall, landing on both hands.

Struck against stationary object

Changing oil under a loader on the gravel when he put left knee down on a rock. He thought that the knee would get better. He had problems off and on and then went in and found out that it was prepatellar bursitis and had to have surgery. Had surgery on Feb. 9th. I'm not sure if this falls under occupational or not?

Over-exertion in pulling or pushing objects

Changing tire on water truck using a breaker bar to tighten lug nuts was pulling up when had a pain in lower back. He didn't report to worker comp. till 8/8/05. They felt that is wasn't compesable and went to court. Just settled that it was worker comp. Missed 33 day and still on restricted.

Stepping off conveyor twisted right ankle.

Was unbridging the bottom of the Jaw and a rock fell and hit the bar mashing his finger against the wall.

Cleaning with the walk behind bobcat under a conveyor when the slab was uneven tipping the machine and smashing his finger on his right hand against a beam.

Struck by falling object

Contusion to the right shoulder when a port a power jack fell while squaring off the sprider on the gyro

Cleaning tail pulley with shovel, he hit frozen dirt which kicked back the shovel putting pressure on his back.

Struck by flying object

A piece of metal popped loose and hit what he was swinging (sq. tubing) and hit his leg cutting his leg open.

Changing wear plates on loader. When taking out the bolts with an impact wrench the bolt spun, cutting his middle finger on right hand.

Pulling motor back with come-along when the hook broke as he was pulling on the handle, causing him to slip and cut his hand on a piece of metal.
