Struck by falling object

Employee was assisting shop welder with hydraulic hammer housing being repaired. Employee used overhead crane to lift one end of hammer housing to enable cleaning and new seal prep. A lifting eye that was tack-welded to the end of the housing, broke free causing the hammer housing to fall to the floor and crush the employee's toes. Employee was wearing safety toed shoes.

Struck by rolling or sliding object

Employee was working on the Jaw Feeder and was removing the Jaw Grizzly Bars. EE was helping to remove the old Grizzly Bars. One of the bars hung up while EE was trying to move it. When the bar broke free, it rolled over and crushed left hand-index, middle, and ring fingers. This resulted in lacerations to all three fingers.

Contact with hot objects or substances

Employee was welding at the Plant #1 Gyro and while doing so a piece of ""slag"" burned through EE's pants and went into EE's left boot and burned the bottom of EE's left foot.

Over-exertion in lifting objects

Employee was moving metal wear bars at the Gyro crusher. While moving the wear bars, EE smashed EE's left-hand ring finger against the stairs and wear bar.
