Caught in, under or between a moving and a stationary object

The locomotive was coming back into the plant when EE had crossed over to the old scales side road. He got out of his truck & started to cross the tracks when the locomotive operator saw him. They heard him on the radio say he had broke the 7 car. A coworker asked another coworker if he could see him & when he looked back he saw him on the tracks. He had been hit by the locomotive.

Struck against stationary object

Working on loader bent down to connect lines on the engine when he put his knee on a bolt that was sticking out from the frame.

Over-exertion in lifting objects

Was removing a big rock from the grizzley with a bar and pulled upward on the bar hurting his back

Struck against a moving object

Bobcat was doing clean up at the hopper when haul truck backing up to dump didn't see the bobcat & bumped the back of bobcat with tire. Employee's neck was hurting.

Struck by falling object

Hammering norback when the material fell and a piece fell on his ankle and foot.

Was putting pin in the hyd. cylinder and got his hand between the cylinder and the cylinder head and smashed his right index finger.

Knelt down to tie wires on the blasting caps for a shot.

Shoveling spills of rock from conveyor belts and shakers, he strained his shoulder. He didn't go to the doctor until 3/20/07.

Stepping off loader twisted knee. He waited to go to the doctor thinking that it would get better.

Lowering a ladder into the gyro to check worn concaves and didn't notice that the safety cable had tangled around his ankle and he released the ladder into the gyro causing him to lose his balance and hurt his left knee.

Tighten bolt on bottom of gear box at a head pulley with wrench when the wrench slipped off a cut his left elbow on a guard.

Was inside shaker changing screens and replacing bed rubber, when he was cutting a piece to install and cut his thumb.

Employee was beating on a piece of metal to close off chute when he hit a rock that came up and glanced his left eye.

Employee was using an air operated impact wrench and the socket flew off and struck employee on a tooth and broke the crown.
