Caught in, under or between a moving and a stationary object

Was backing the take up off with 3/4"" impact wrench when the bolt broke causing the impact to smash his fingers against a piece of metal on the frame.

He was parking the loader in the equipment lot to get on to another piece of equipment. When he came down the ladder and placed his foot on the ground twisted his right ankle.

Installing cylinder pin in 769 water truck, while pinning cylinder in truck he over-strained his muscles in confined space.

Contact with hot objects or substances

Was cutting metal when a fireball fell on his blue jeans over his left knee causing a burn on his knee. Didn't go to the doctor till 7/30/08.

Fall from machine

Had patch holes in a rail car and was climbing out when he slipped and fell hurting his back. It was his three day.

He was replacing cutting edges on a loader and strained his groin while picking up a cutting edge.

Caught in, under or between a moving and a stationary object

Changing roller bracket got his hand caught in the bracket when it fell off the conveyor. The accident was on 4-9-08 but he didn't go to the doctor till 4-14-08
