EE was working on a track hoe bucket and was hitting the bucket with a 4lb hammer when a piece of the hammer broke off going into left upper thigh.

EE was shoveling out a feeder when EE smashed EE's finger between the shovel handle and the frame of the conveyor.

Struck by falling object

Threw a piece of rip over car and it landed on my left index finger instead of falling off the side of rail car. Cleaning out rail cars
