Caught in, under or between a moving and a stationary object

Trying to put a rail bolt into a shaker when he slipped on some ice and his foot went between the floor and shaker.

Over-exertion in pulling or pushing objects

Unbridging the feeder when he sprained the muscle in his abdomen. He didn't report until 11/09/2009.

Struck against stationary object

Employee was using wrench to remove hose and it slipped off of nut and left hand hit bottom of engine block. Laceration to 2nd knuckle of hand.

Struck by falling object

Metal sheave counterwight came loose from a come-a-long and fell across his lower leg and foot.

Over-exertion in lifting objects

Was lifting gear on the side of feeder, putting on cover of the gear on the other side, and standing on unlevel ground. Accident was on 7/29/09 he didn't ask to go to the doctor till 8/13/09.

Struck by falling object

He was under the gyro crusher cutting bars out, when a bar fell and hit his foot, right behind the steel toe of the boot.

Over-exertion (Not Elsewhere Classified)

Was changing screen on 6x20, slipped and hurt his back. Didn't go to the doctor till 1/13/09.

Struck by flying object

The weld broke loose on piece of metal and the porta-power came up and hit him in the nose.
