Bodily reaction, (Not Elsewhere Classified)

He had been standing all day running the plant when he turned wrong & hurt his back. He went to the Dr. on 6/10/13 the Dr. said it was a strain and employee went back to work. He was still having some trouble 7/11/13 they scheduled an MRI. He seen surgeon on 7/22/13 & decided he didn't want surgery. He then went back on 8/8/13 & he decided to have the surgery on 8/28/13.

Struck against stationary object

Was changing v-belts when pipe wrench slipped causing his hand to go around the shive cutting his right hand little finger

Struck against a moving object

THE TRUCK DRIVER RAN OVER A BUMP IN THE HAUL ROAD CAUSING HIM TO BE JARRED INSIDE THE TRUCK WHICH CAUSED HIM BACK PAIN. THE DRIVER WAS SEEN BY A PHYSICIAN AT WHICH TIME HE WAS DIAGNOSED TO HAVE A BACK STRAIN. DRIVER WAS GIVEN AN IMMEDIATE RETURN TO WORK STATUS WITH ""NO BENDING GREATER THAN 6 TIMES PER HOUR"" RESTRICTIONS. FOLLOW UP APPOINTMENT SCHEDULED FOR 9-18-2013

Employee was stepping backwards off ladder on loader and his knee popped

Cleaning the scales and he jump under the scales landing on small pile of rock twisting his ankle

Struck by flying object

He was grinding inside the main frame liners in the simmon crusher when a piece of metal got in his right eye. He was wearing all of his PPE, safety glasses, and face shield. He also washed his eye out several times thinking that he had gotten. On Friday March 1st is when he when to the doctor.

He was cutting a piece of flashing when the blade came out of the flashing cutting his thumb.

After exiting the excavator he started walking he slipped so to catch himself he put his hand out and a rock cut his hand.

Caught in, under or between a moving and a stationary object

He was adjusting v-belts on the main feeder with a wrench when the wrench came loose it smashed his finger between the I-beam and wrench.
