Electrician was helping scoop operator change batteries. EE had female end of jumpers and was connection it to scoop when wiring pulled loose inside bell creating an arc. Palm of his left hand was burned. Was treated and released at CAMC. He returned on his net shift. He followed-up at St. Marys Burn Clinic. The doctor removed dead tissue with no infection and released him.

Power knocked on CM (JM6432). Operator and scoop man were tightening the waterline swivel near a damaged place in the cable. Someone reset the circuit breaker and cable went phase to phase creating an arc. The scoop operator may have received. a shock when the arc occurred as he stumbled away from the cable. EE was kept in the hospital two days for observation.

Member was helping re-enter hi=line cable into the #5 belt head kva. member was holding the lead out of the way for the electrician. A malfunction in the #1 box allowed that particular lead to become energized, causing the member to receive a shock.

Member received burns when he came in contact with energized electrical components while performing preventive maintenance on electrical installation.

EE was installing a fuel tank on a tractor inside the truck shop, when lightening struck a nearby transformer on a power pole. The electrical charge traveled down the power pole and disbursed out through the ground, making contact with EE. He was transported to CAMC General Hospital, evaluated, given fluids as a precaution and released back to duty.

EE was working between shifts to install belt hangers. The power had knocked. EE sat down on the T-bar of the roof bolter and said it shocked him. He was transported outside and then to the hospital via ambulance. MSHA, State, and Company officials investigated all aspects of electrical systems at the mine site and could not determine the cause of the accident.

Injury was changing a valve insert in Komatsu 930 E Rock Truck when a bolt of lightening struck the ground approximately 60 feet from him. The electricity traveled through the ground and entered his body at the left shoulder and exited through both of his feet causing burns. He was transported to CAMC General Hospital, evaluated, monitored and released.

The injured employee, an outby electrician, was changing a blown fuse using a pair of channel locks. As the fuse contacted the connections it arced and blew, resulting in burns to the posterior side of both hands. He did not cease work at that time but later went to the ER and was diagnosed with first degree burns and has been placed on restricted duty.

EE was attempting to reset a 480 volt, 225 amp breaker. When he did it blew up causing a 2nd degree burn to the top of his right hand and a first degree burn to the side of his face and ear.

Employee 1 break across from 3-D belt head helping start the forklift. When he tried to boost the battery it exploded causing injury to his eyes. Taken to E.R. for treatment, Dr took him off work until follow-up 11/24/08.

EE was hooking up a bonder to the scoop batteries. There was an arc, which burned this right hand, middle, ring, and index finger.

While working on a 55 KVA power box, EE came in contact with high voltage, when he raised the top lid.

Employee was wiring and mounting control devices in an out of service motor starter. He was mounting control devices in an empty compartment above the starter when he apparently drilled through the floor of the compartment into an energized buss tap.

Individual came outside after shift and remembered he had to deliver a contractor back to the section for the miner. When returning to section he went to the miner and bent down to look into the panel. When the contractor went phase to phase causing an arc flash burning his face and hands.

Employee was hooking highline into mouse house, 4th piece of cable during a lighting storm; lightening struck close by coming through cable causing cable to energize and shock him.

Employee was putting water in his cap light; while removing the water bottle the water or acid from the battery splashed in his eyes.

EE was hanging the roof bolter cable when he received an electricla shock, MSHA and OMSL were notified and investigated accident. Member was taken to hospital and evaluated and received a pain shot. No further treatment was needed.

Employee was troubleshooting the contactors, when the miner was energized the contactors blew up, causing flash burns to ee's eyes.

EE was helping assist the foreman in hanging a D-Box cable whe EE grabbed the cable he came into contact with a bad place in the cable and was shocked. EE was checked by Ambulance personell and was deemed okay with no medical problems.

Electrician was trouble-shooting IGBT tower and failed to allow capacitor banks to discharge completely and was shocked by the capacitor banks.

Battery exploded and a piece of it hit employee in forehead, required stitches.

EE was preparing to move a anchor for a shuttle car when he received an electrical shock. MSHA and OMSL were notified and investigated the accident. He was admited to hospital for observation he was kept until 12-15-05 at which time he was seen by a specialist and released to return back to his normal job.

EE was hanging the cable of a joy 21 shuttle car and came into contact with electricity at a splice.

Member noticed that his pants were wet and his cap light battery was leaking. Member received 2nd degree chemical burn but continued to work until 10/6/05.

EE WAS OPERATING RT. CONT. MINER IN THE #5 ENTRY & RIBBED OUT THE STANDOFF DAMAGING THE CABLE & KNOCKING THE BREAKER AT THE POWER CENTER. ATTEMPT TO RESET THE BREAKER, POWER OUTSIDE. ELEC. WAS INFORMED OF THE SITUATION, EXAMINED THE CABLE & TRAVELED TO THE POWER CENTER TO DE-ENERGIZE THE CIRCUIT & BLOCKED & TAGGED THE WRONG CATHEAD. POWER WAS RESTORED & HE GOT ELEC BURNS

MEMBER AND FOREMAN WERE PULLING CABLE OFF A 21 S/C REEL TO ALIGN LEVELING ARM WHEN MEMBER CONTACTED A CUT PLACE IN CABLE RECEIVING AN ELECTRICAL SHOCK. CAUSING MEMBER TO FALL DOWN INJURING HIS BACK RIGHT HIP.

EE WAS WALKING BY THE PANEL OF THE FEEDER AND STATED THAT HE SLIPPED/TRIPPED AND CAUGHT HIMSELF WITH HIS LEFT HAND IN THE PANEL, RESULTING IN AN ELECTRICAL BURN. EE WAS TRANSPORTED TO CAMC GENERAL AND TO WESTERN PENNSYLVANIA HOSPITAL WITH 3RD DEGREE BURNS. THERE WERE NO WITNESSES TO THE ACCIDENT.

INJURED SUFFERED ELCTRICAL BURNS WHILE GETTING VOLTAGE READINGS ON #1 CONTINUOUS MINER RECEPTACLE. HE WAS TESTING WITH A FLUKE MEGOHMMETER MODEL 1520 THAT WAS RATED FOR 1000 VOLTS.

EE WAS ASSISTING WITH CHANGE OUT OF A TRANSFORMER.DISCONNECT WAS LOCKED & TAGGED OUT. THE DISCONNECT MALFUNCTIONED, LEAVING TRANSFORMER ENERGIZED. EE CAME IN CONTACT WITH POWER.

CONTRACTOR WAS JOGGING TUNNEL BELT WHEN BREAKER BLEW UP & EXPANDED DOOR WHICH CAUSED HIM TO STOVE HIS FINGER UP OR MAYBE EVEN OVERLAPED HIS JOINTS ON RIGHT THUMB.

MEMBER WAS MANUALLY DISCONECTING THE POWER ONT HE 002 SECTION POWER CENTER. HE WAS LOOKING THU THE PLEXIGLAS WINDOW TO SEE IF THE BLADES WERE DISCONNECTED. HE RECEIVED A FLASH BURN TO HIS EYES REQUIRING USE OF PRESCRIPTION MEDICATION.

SCOOP RAN OVER OLD SCOOP BATTERY CELL AND SPRAYED EE WITHE BATTERY ELECTROLYTE CAUSING 1ST DEGREE BURN.

EE RECEIVED SHOCK AFTER UNHOOKING A THUMPER FROM FINDING A GROUND IN MINER CABLE. HE SAID HE FLIPPED TOGGLE SWITCH OFF, DEPRESSED ACTIVATOR WHEN HE FELT A SHOCK. HE WAS TAKEN TO HOSPITAL WHERE SEVERAL TEST WERE PERFORMED. UPON INVESTIGATION THE THUMPER KNOB WAS FOUND ON HIGH. THIS REPORT IS FILED DUE TO AN MSHA VIOLATION. MANAGEMENT BELIEVES A MEDICAL CONDITION KEPT EE FROM WORK.

EE WAS BACKING THE SCOOP DOWN THE #2 ENTRY TOWARD SECTION. HE BACKED INTO A CROSSCUT TO TURN AROUND AND STRUCK A SET OF SCOOP BATTERIES THAT WERE ON CHARGE. WHEN HE GOT OUT TO CHECK HE PLACED HIS HAND ON THE SCOOP CANOPY AND RCVD. A SHOCK.

EMPLOYEE WAS MOVING CONTINUOUS MINER CABLE WITH HIS RIGHT HAND WHEN THE CABLE WENT TO GROUND ANDEXPLODED. EMPLOYEE WAS TAKEN TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS EXAMINED AND RELEASED.

EE WAS MOVING SHUTTLE CAR CABLE OUT OF ROADWAY SO SCOOP COULD GET THROUGH. HE ALLEGED WHILE HANDLING CABLE, HE RECEIVED A ELECTRICAL SHOCK TO HIS LEFT ARM CAUSING IT TO LOOSE FEELING.

EE UNPLUGGED A SHUTTLE CAR CATHEAD FROM THE SECTION POWER CENTER BUT DID NOT TRIP THE BREAKER. WHEN HE ATTEMPTED TO PLUG ANOTHER CATHEAD INTO THE SAME RECEPTACLE THE CATHEAD EXPLODED. EE SUSTAINED SUPERFICIAL FLASH BURNS TO HIS FACEAND ARMS. SAFETY GLASSED AND GLOVES WERE WORN PREVENTING INJURY TO EYES AND HANDS.

EE WAS LEANING OVER TO CLOSE BATTERY LID ON LOCOMOTIVE. A LOCK ON HIS BELT TOUCHED THE BATTERY CAUSING AN ELECTRIC FLASH WHICH BURNED HIS LEG.

WHILE TROUBLE-SHOOTING A ELECTRICAL PANEL ON A CONTINOUS MINING MACHINE UTILIZING A VOLT/OHM METER. THE METER FAILED AND EE SUSTAINED BURNING OF 1 & 2 AND DEGREE TO BOTH HANDS.

EMPLOYEE WAS ATTEMPTING TO REPAIR A LOW VOLTAGE TRAILING CABLE WHEN HE CUT INTO THE CABLE, HE CAME INTO CONTACT WITH AN ENERGIZED CONDUCTOR.

EMPLOYEE WAS BLOCKING THE SECTION FEEDER AT THE SAME TIME THE MAINTANENCE CHIEF WAS TROUBLE SHOOTING THE FEEDER PANEL. EMPLOYEE HAD BEEN WARNED TO STAY CLEAR. WHEN EMPLOYEE RAISED UP FROM THE FEEDER, HE USED HIS HAND TO PULL HIM- SELF UP AND CONTACTED THE FEEDER PANEL WHICH RESULTED IN AN ELECTRICAL SHOCK.

EE REMOVED THE RIGHT RIB CATHEAD FROM THE SECTION POWER CENTER LOADING TO FIND A GROUND ON THE CABLE HE WENT TO RIB WHERE REFUND THE GROUND. AS HE WAS LEAVING HE SLIPPED AND FELL CATCHING HIMS ELF WITH LEFT HAND AGAINST RIB RECEIVING A SHOCKFROM THE TESTED TRANSPORTED TO LOGAR GROUND FOR OBSERVATION EKG NO ABNORMALITIES.

EMPLOYEE WAS TROUBLE SHOOTING CATHEAD CONNECTION WHEN IT FLASHED. THE FLASH CINGED HIS EYES AND FACE.

EE WAS HANGING ROOF BOLTER CABLE WHEN HE CAME IN CONTACT WITH A DAMAGED PART IN THE CABLE. EE SUSTAINED AN ELECTRICAL SHOCK. HE WAS TAKEN TO WILLIAMSON MEM HOSP WHERE HE WAS KEPT OVER NIGHT FOR OBSERVATION. EE RET'D TO WORK NEXT SHIFT

EE WAS HANGING PINNER CABLE. A PIN HOLE IN THE CABLECAUSED IT TO ELECTROCUTE THE EE. EE SUSTAINED NO BURNS, BUT COMPLAINED OF CHEST PAIN AND DIZZINESS. HE WAS HELD OVERNIGHT IN THE HOSPITAL FO R OBSERVATION.

EE WAS UNLOADING AMMONIUM NITRATE INTO BIN WHEN LIGHTING STRUCK THE GROUND(500' EST) FROM BIN. EMPLOYEE REACHED TO TURN PRESSURE GAUGE ON TRUCK AND RECEIVED ELECTRICAL SHOCK FROM HOLDING PRESS URE GAUGE.

INJURED WAS VISUALLY CHECKING CONTROL WIRES AND MAIN BREAKER WHEN THE CONTROL WIRES GROUNDED. THIS CAUSED AN ELECTRICAL ARC RESULTING IN A 3RD DEGREE BURN ON THE LEFT HAND. HE WAS TRANSPORTED TO HOSPITAL WHERE HE WAS TREATED AND RELEASED.

MEMBER WAS IN THE PROCESS OF MAKING A BELT AND POWER MOVE ON THE 001 SECTION. HE WAS STANDING IN THE BELT ENTRY BY #2 SHUTTLE CAR, HE PICKED THE CABLE UP AND DROPPED IT AGAINST THE RIB. THE CA BLE BOUNCED BACK ON TOP OF HIS RIGHT BOOT CAUSING HIM TO RECEIVE ELECTRIC SHOCK. MEMBER WASWEARING LEATHER METATARSAL BOOTS WHICH WERE WET. HE WAS TRANSPORTED TO HOSPITAL.

MEMBER WAS WALKING BY #3 S/C WHEN THE ELECTRICIAN TRIED TO PUT THE BREAKER IN. THE S/C HAD A GROUNDED CABLE AND THE MEMBER HAD HIS HAND ON THE S/C. HE WAS GIVEN AN INITIAL SHOCK. HE WAS TAKEN TO HOSPITAL FOR OBSERVATION & RELEASED. HE RETURNED TO WORK THE NEXT DAY.

EE STATED THAT HE WAS SHOCKED WHILE STANDINB BESIDE THE R/B IN ABOUT 2" OF WATER. HE HAD HIS LEFT HAND ON AN INSTALLED ROOF BOLT, BUT WAS NOT TOUCHING THE ROOF BOLT MACHINE NOR THE ROOF BOLTER CABLE. HE COMPLAINED OF CHEST PAIN, WAS TRANSPORTED TO HOSPITAL. HE COULD NOT SEE A SPECIALIST UNTIL 7-18 & AT THAT TIME HE WAS RELEASED TO WORK.
