Employee was bolting on the right side of the continuous miner when a piece of slate fell out of roof and struck employee in the right shoulder. Rock was 6"x8"x4".

Employee had pulled walk thru roof bolter in #6 entry to bolt and set the T-bar on the roof bolter. The employee had grabbed a couple of bolts to lay in the bolt tray. EE placed the bolts down in the tray when a piece of undetected draw rock fell striking EE on the right ring finger resulting in a fracture to the end of the finger and required stitches.

Employee stated, was roof bolting in 5R c/c when struck by loose rock which fell during the bolting cycle. Rock impacted personal canopy and then pushed employee to ground. EE was transported to surface by EMTs and taken to RGH ER. EE was treated and released but returned to doctor 2 days later and is away from work until 6-3-16

The injured employee was installing a rib bolt when a piece of draw rock fell from the top hitting EE on upper back and pushed EE to the ground. The employee was taken to the Logan Med Center ER and given a full release to return to work, EE went to Dr on 05-31-2016 and was taken off work for a possible pinched nerve, the employee started missing work on 06-01-2016.

While waiting for off side operator to finish putting up inside bolt injured employee was reaching across the top of the inch tram levers on the bolter to get some roof bolt plates. While reaching over, a piece of draw rock measuring 81" x 42" and 0-3" in thickness fell, driving employee into bolt. EE's lip was cut from the impact of hitting the guard over the inch tram levers.

The employee was in the process of removing a rock from on top of the Joy continuous miner when a piece of rock measuring approximatley 4'L X 28" X 1 to 2 inches thick fell from the mine roof striking the employee on the right arm resulting in a laceration requiring 4 sutures.

Employee stated after T bar was set, EE walked forward to swing boom outward to install EE's rib bolt. A piece of draw rock 6'L X 3'W X 2"T fell on EE's back forcing EE to the ground. Once EE caught self with EE's hands, the rock slid down EE's back striking EE's right hand causing bruising. Employee was transported to doctor for X-rays which were negative for any fractures.

Employee stated EE was operating CM when a piece of brow rock fell out striking EE across the shoulder driving EE to the ground. Employee finished EE's shift and did not seek medical treatment at this time. EE continued to work and sought medical treatment on 10/26/17 and given time off. EE began missing work on 10/27/17

Employee was bolting top in #2 face when a piece of draw rock, approximately 60" X 21" X 1" - 1 1/2" thick dislodged, striking him on the head knocking him to the ground. It was reported to the Safety Department on 12-16-15 Employee was having physical therapy for his injury.

While bolting, employee was struck in left hand by a piece of draw rock that fell from roof.

While installing the inside bolt in the first row in the Number 7 entry the employee had set the starter steel and as he was removing his hand a rock fell from the top landing on his hand smashing it in the Drill Pod resulting in a laceration and ten (10) sutures to his right index finger.

Employee states while bolting top, a piece of draw rock fell out striking him on the left forearm. The starter drill steel stayed in the top and he was getting ready to add drive steel when the starter steel fell out. While reaching to catch the steel, draw rock fell from top striking him resulting in 13 stitches.

Employee was scaling loose rock after setting the ATRS against the top when a piece of loose material dislodged from the roof behind him. The material pushed his hard hat down hitting his safety glasses causing a small cut on the bridge of his nose and caused him to bite his lip which required 3 dissolvable stitches on the inside. The cut on his nose was glued.

EE was roof bolting when a 2 1/2' long rock fell from roof, striking ee's Lt arm. Even through ee was wearing all PPE, fall still caused a laceration on ee's Lt forearm that received 6 sutures. This is being reported as a medical treatment (NDL).

After finishing the cut, employee was struck and injured by a rock.

Employee was traveling along the track on 4belt. When a piece of rock (measuring 4'X 2.5' X4") Fell from between the bolts and struck the Employee in the right shoulder knocking him off the ride. Injured went hospital and received a Diagnosis of Injury to the head, sprain of shoulder, contusion and a Concussion.

While Employee was loading a shuttle car with the miner; a piece of rock fell out between a row of bolts striking Employee on the back of neck.

Bates Contractor was drilling hole for rib bolt. While stopping to install his wrench to continue drilling the hole, he reached over the drill pot and material from the brow sloughed out hitting his nose resulting in a laceration requiring 14 stitches.

Employee was drilling last bolt hole in the cut when an undetected slick sided rock slipped out hitting the top of the canopy breaking with one piece of the rock striking him on the left side of his back resulting in a contusion and 1.5" long laceration with no stitches required.

While employee was drilling a hole to install a roof bolt, a piece of draw rock became dislodged, rotated down the steel, broke apart and was projected toward employee striking him in the nose, causing a laceration requiring stitches to close.

Bates Contractor was putting up second bolt and rock fell out and struck right arm.

Employee was standing alongside the roof bolter when a piece of draw rock fell striking him in the back, pushing him into the bolting machine. The rock measured approximately 3'long x 12" wide x 0-12" thick.

Employee was operating a roof ranger 2 bolter in the 2 left crosscut. While he was installing the outside bolt of the second row on the operator side a piece of draw rock measuring 24" long 17" wide 1" 1/2 in thickness came loose from the roof and slid down a pie pan striking the employee's left hand and fingers between the rock and the side of the bolter.

Employee was operating a continuous miner in the No.1 entry face during a clean-up of the left side of the entry when a piece of draw rock measuring 67" long 27" wide and 0 to 6" thick fell from between the bolts and the right rib line striking him on the back side of his right ankle. Employee was taken to the hospital where X-Rays revealed a broken bone.

Employee arrived at the section to restock the CO#247 roof bolter. As he was walking away from the bolter after checking what supplies were needed, a piece of draw rock fell from the roof striking employee on his right foot. The resulting accident required the complete amputation of all 5 toes on the right foot due to their damage from the falling draw rock.

As Employee was walking around scoop a piece of rock fell striking Employee on the neck and back. This accident has been under internal investigation.

While installing a 5' torque tension bolt a piece of draw rock approximately 19.5" long, 13" wide, and 4" thick fell onto employees' foot.

Bates Contractor was operating the operator's side of roof drill. While starting the hole with the drill steel a piece of rock broke loose and fell out of the top hitting employee in left arm, cutting arm.

When employee went to change his drill steel out, a piece of draw rock fell hitting him on the right wrist. He had a simple fracture to the wrist.

Rock fell out from between straps striking EE on head and shoulder. EE was taken off work on 1/8/2015 due to spasms and limited motion.

Employee set steel against the top and was moving hand away to start the rotation. A rock that measured 2.5' long X 6" thick X 1" wide fell catching hand between rock and the drill pot.

The injured was making up a roof bolt along side the continuous miner when a piece of coal (10" by 11" by 8") fell and struck him in the wrist.

Employee was troubleshooting the RCM when a piece of draw rock fell striking Employee in the head. He was treated at Beckley Appalachian Regional Hospital where he received nine (9) staples in his head.

Employee states while drilling a hole, draw rock fell from the top striking him on the left side of his face and left eye area.

Bates Contractor disengaged the T-bar from the mine roof, when a piece of draw rock dislodged from between the rib and the outside bolt striking him on the left ankle. The contractor (5UF) submitted a 7000-1 on 10/27/14, however it should have been submitted under this MSHA ID.

As Employee was operating continuous miner a rock fell striking Employee on the neck causing pain in his lower back. Employee was treated at Raleigh General Hospital and was diagnosed with a compression fracture of the L2 and L5. Employee has a follow-up appointment with a specialist.

Employee was driving a mantrip from the outside to the section, when a rock fell from the top and struck in the head / shoulder area knocking his hard hat and glasses off of him. He was sent by Ambulance to SMH and was granted a full release. He informed Mine Management that due to the injury he received on 10/21/14 that he would be going off work on 3/16/15.

EE was standing behind miner on Lt side. Miner was 2' from corner. Operator called for the jacks to be set. EE walked between the corner and miner. EE hit the pie pan which dislodged a piece of rock/coal and it struck EE on the back. EE began lost days on 10/24/2014.

While bolting a piece of rock fell and smashed Employee's left middle finger against the pot. Employee was treated at Raleigh General Hospital where he received seven (7) sutures.

Working on the #1 roof bolter, he was on his knees when a rock fell out in between 2 bolts striking the employee in the head. The rock then broke and hit the employee in the hand causing a laceration that required 2 sutures.

A piece of draw rock fell from the top striking employee's hand causing severe laceration, and break to hand.

While Employee was standing in #4 entry top brow fell off striking Employee in both legs. Employee was transported by EMS to CAMC General Division. (Resulted in fracture of lower right leg.)

A rock that measured 34" in length and 20" in width with a thickness of 4" fell from between two roof support straps striking employee on the head and left shoulder.

As Employee was hanging roof bolter cable a piece of draw rock fell striking Employee on his back and left shoulder. Employee was treated at Raleigh General Hospital.

Employee was bolting in #6 entry, putting up the last row of bolts in the face. The hole was drilled and the employee was putting the glue in the hole when a piece of rock fell glancing him on the back of the head resulting in a laceration.

After roof bolting machine was pulled up to install next row of bolts & after ATRS was set, IE swung his head out & placed drill steel into head in preparation to drill outside hole, when a piece of roof rock(18"x12"x1"-2" thick) fell striking canopy then striking his left hand causing laceration.

As Employee was walking out of the entry a rock fell striking Employee on the head and ankle. Employee was treated at MedExpress where xrays were taken and received ten (10) staples due to the laceration on his head.

Employee was rolling up curtain when rock fell and hit his shoulder and knocked him down. Employee was treated and received full release at Logan General Hosp. Continued to work until 08/19/2014 where he was seen by Dr.for back pain and an MRI is scheduled. Employee is off waiting on MRI and results. This is being reported due to the Pt.50 10 day rule.

Employee was helping left side bolter on CM make up bolts when a rock (3'9" l, 2' w, 10" thick)struck him in the head, neck, right shoulder and forearm, and left hand causing two fractured fingers, several lacerations to the fingers, and severed the tip of his left thumb. The rock fell out between straps. The roof support was installed according to plan.

Employee was bent over, cleaning rock dust from the rib with a hammer when a rock fell striking him on the top of the head, jamming his neck. The Maxxim Shared Services employee began receiving therapy on 8/29/2014.
