The employee was using a dial indicator to check runout on a motor sheave. After positioning the dial indicator, the employee manually pulled on the belts to rotate the sheave. During this process, the employee's finger became caught between the belt and sheave.

IE was closing a 15' x 15' raw mill door with another employee and the IE right middle finger was caught in between when the door (as it rebounded) and the nut on the locking mechanism. causing a smashed finger

The worker was trying to shut an entry door that broke and the door handle hit them in the right side of their ribs. Didn't meet reportable criteria until 7/3/2026

Employee was setting brake on rail car when felt discomfort in shoulder. Employee refused medical care until 7/7 and was referred to a specialist. Employee was seen by specialist on 7/29 and Diagnosed with shoulder tear.

A TDF attendant was unstacking tires. The EE pulled a tire off the stack, causing dust to hit EE's face, and enter EE's left eye causing irritation. The EE wore safety glasses, first aid was administered EE reported discomfort on 5/7 and EE went to and urgent care facility. No foreign body was discovered, EE was given no restrictions. The doctor prescribed antibiotic.

The employee had CKD dust in EE's boots, which caused a chemical burn. Because EE wasn't able to wear boots, EE had restrictions on work starting on Apr 14.

The employee was cleaning up used vac hose (6" corrugated piping) when they pushed some filled piping over the the toe plate and as it was going over the end whipped up and slung cement dust into their eyes. Prescription drops prescribed.

While clearing out full mechanicals on K22, PA had some CKD flush out through the port they were poking through. The dust contact on the PA's skin caused some irritation; later, the PA was diagnosed with second-degree burns in the neck region.

Finger was caught between two bricks. Employee had a fracture with stitches.

Employee was opening and removing an inspection door on a LOTOTO clinker cooler fan. The door fell forward onto the EE's hand breaking a finger

EE was moving bricks and when one fell at a time, while trying to accommodate the other bricks, the little finger on EE's right hand got caught in the middle of the bricks.

On 9/22/2025 While attempting to open a door that was stuck employee felt a pain in their elbow. Employee chose not to be evaluated at the time and do first aid measures on their own. On 10/27/2025 employee went to the clinic was seen by a Doctor who diagnosed employee with Tennis elbow and prescribed a steroid pack.

Employee was flipping the cutting edge on front end loader and received a laceration to left hand palm from bolt head.

Burn to foot that became infected.

While cleaning material up from a completed job a part that was unsecure moved striking the left middle finger tip causing a laceration requiring stitches.

Employee was tasked to set the classifier. During the lift, EE's right thumb got pinched. EE was taken to the local ER and treated.

Employee tripped on a beam and reported sore pain in ankle and calf, at initial report thought to be a sprain. Later in the week, employee was diagnosed with leg fracture.

Employee sustained a sprained finger.

While replacing a return roller, the employee loosened the bolts and the roller shifted and smashed the ring finger resulting in a laceration that needed medical attention.

The employee was working on the TDF dock, where they experienced a foreign body sensation in left eye while they were offloading tires. The employee did not report the incident until 6/23 until after visiting the ER where they were given an RX antibiotic.

Employee suffered chemical burns after work poking on 5 reject dust containing CKD. EE had initial skin irritation and didn't report the event. After the injury worsened on June 9th, they reported to their supervisor. On June 12th, the medical report stated it was a second-degree burn.

Employee suffered a shoulder strain that required medical treatment while opening mill body doors.

While trying to install piping for heat exchanger, the pipe shifted and the individual had hand in the proximity of the pipe. The employee's left, ring finger suffered avulsion at the tip from the pipe and pinch point of a piece of support steel/gusset

Employee was exposed to caustic fluid during clearing of a plug and cleaning of the area to their feet when their boots and socks became saturated with the fluid causing chemical burns.

An employee lifted a bucket of dust to top of handrail and as they lowered the bucket of material, the employee felt a pull or strain in their right shoulder. First aid until 3/14/2025 when they received light duty and script meds.

Injury occurred to a customer (Xcel Bulk) truck driver. When trying to pull up on a Customer Service SafeRack safety cage, the injured truck driver felt some pain in their right shoulder.

Employee's hand made contact with discharge water from nozzle of pressure washer wand that the employee was operating.

While moving a ladder, employee knocked dust off of light. Dust got into employee's eyes.

Employee dropped roller on gloved hand fracturing middle finger and causing bleeding.

Worker was installing transformer on a wall and it fell and hit EE's left hand causing laceration.

Employee was reinstalling a port cover to an access port on the 565-SC1 screw conveyor (which was locked out), and the employee reported that the cover sucked in quickly, pinching EE's finger between the port cover and the port opening.

Employee was loading a tire on a monorail hook and felt a pop above their belly button.

Employee was installing idler pulley when the pulley slipped, cutting their index finger.

Employee sustained a laceration to their left forearm from a dial pen that protruded further than the gate valve it was affixed to.

Pulling up on a bag house bag, when it broke loose the employee hit their face with their hand chipping a tooth.

Employee was shoveling and felt pop/pressure in abdomen. Employee diagnosed with hernia. Doctor restricted employee to limited lifting until follow-up.

Maintenance employee was lifting a piece of shaft stock with the help of another technician.

Employee received a diagnosis of a bicep tear on July 18th, related to loading an empty gas cylinder on a cart.

Employee was working on a flapper of a pneumatic pump, and reported feeling a strained muscle in shoulder. Employee continued normal shift work, after reporting the incident to employee's supervisor. On 8/9 employee was placed on modified duty resulting in reportable incident.

Employee was trying to remove a dust collector bag/cage from a locked out screw conveyor and they leaned their torso against the metal frame of the equipment, when they felt a pop in their ribs. Employee didn't feel any pain but finally reported the incident at 7:00 am on 7/1/2024

The storekeeper was moving idlers in the storage area. One piece of equipment moved suddenly and pinched the storekeeper's fingers.

The worker was transporting buckets with wet clinker and suffered superficial chemical burns due to the contact with the caustic material.

Injured employee was working in a small group, removing bags/cages from a dust collector compartment for 3 days. The employee felt sore in EE's left shoulder at the end of day 1 but was feeling increasing pain and discomfort in EE's left shoulder. The employee decided to seek doctor evaluation on 5/16 and did not report to EE's Supervisor that EE was hurt until 7am on 5/16.

Employee cut their left ear on a piece of metal while cleaning in the area, resulting in 7 stitches.

While moving pallets a fork came off the forklift. While trying to put the fork back on the EE got their ring finger (right hand) pinched between the fork and the mast.

Employee was moving a drum and the drum cut the left thumb.

The employee was moving oil drums and noticed a bulge starting (At this time EE didn't inform the mine operator) After 13 days EE went to a doctor where EE was diagnosed with a hernia that needed surgery. The mine operator was informed about the need for surgery on 05/13/2024.

EE was coming out of the scrubber when EE's lanyard hook struck EE in the front tooth.

Worker felt pain in right elbow while lifting a piece of scaffold from ground level.

Employees were tempting to install expansion joint and place the chains to rig it while employee was tidying it up it came lose and caught employee's hand.
