EE was assisting the plant welder, with fabricating brackets for the bin gate clam shell. While using a drill press and grinder small metal particles collected on the sleeves of the EE shirt. EE wiped EE's face with EE's sleeve contaminating EE's eye with the small metal particles. EE did not seek medical treatment until 5/12/2026.

This caused operator to be pushed into the mast and cut EE's arm. Operator has since been retrained on proper loading and unloading of drill pipe for Gill Beetle Drill on 4-28-26
On April 13, 2026, employee experienced a right elbow strain disconnecting a loader shroud. Employee attributed it to an awkward position while using an impact gun. Reported on April 16, 2026, received an initial first aid assessment . Due to ongoing pain, a medical evaluation on April 28, 2026, diagnosed medial epicondylitis, leading to modified duty.
EE was attempting to lift handrail which had been tack welded to the floor. When they lifted the item with the crane, the steel floor lifted with it, they stepped forward to check why the handrail was not lifting and their right foot ended up underneath the floor plate as the weld failed and the plate fell onto their foot causing a fracture.
A 600 psi air hose on the drill ruptured causing the window to shatter. The operator suffered a laceration on the top of their head requiring 3 staples to close.
Employee was welding when a piece of slag got into their eye causing irritation.
Department of Agriculture inspector had Left Pinky Finger stuck between weights cart and weight that led to 7 stitches
While changing cutting edges on a 992 loader bucket, employee reached under to check if bolt was spinning or not. While grabbing the bolt the employee accidentally pulled the trigger on the impact gun and turned the bolt causing it to slice their finger.
Employee was using a 12-ton bottle jack to lift forms in order to adjust a wheel position for form reduction. The employee's left hand was positioned between the bottle jack and the formwork. Upon engagement, the forms shifted, pinching the employee's left index finger between the form and the jack
USING EXTRACTOR BIT TO DRILL OUT BROKEN BOLT, BIT SNAPPED CAUSING HAND TO SLIP DOWNWARD AND CUT TOP OF HAND
Failure of installation contractor rigging equipment allowed rotating bucketwheel assembly to strike employee's leg.
Employee was doing daily inspection on jaw crusher, when a haul truck dumped its load at the top of the ramp. The material surged from the primary into the grizzly feeder and then a rock came out and struck employee on the back of the hardhat.
ON 9-22-25 EMPLOYEE REPORTED HAVING A FOREIGN BODY IN EE'S (R) EYE FROM TORCHING HOLES ON A NEW CLAD INSTALL ON 9-19-25. THE FOREIGN BODY WAS REMOVED IN THE EMERGENCY ROOM ON 9-22-2025. EE WAS PRESCRIBED EYE DROPS AND WAS RELEASED TO FULL DUTY SAME DAY.
EE was using a battery powered drill when the drill bit got caught on the steel causing the drill to quickly spin around. This caused the bottom side of the drill to flip and impact EE at the top of their safety glasses, resulting in a laceration above their right eye.
Employee was replacing roll liners in a 992 G bucket at Clear Creek Wed morning. While grinding the bucket to prep for new liners to be installed metal shaving from the grinding had gathered on top of EE's grinding shield. Once EE stood up and raised EE's shield the material fell down EE's forehead and behind EE's safety glasses.
Air conditioner had a major failure and leaked freon fumes into cab of drill for four hours before driller started noticing symptoms. ER was decided upon. Return to work given after 2 days rest. Air conditioner was completely gone over and rebuilt.
Employee was welding liners into a discharge chute when they felt something hit their face. Later in the day they realized a piece of debris was in their eye. They sought medical attention the next day and the debris was removed.
The accident occurred when employee was attempting to get a hammer disassemble in a set of jaws. They put a spacer piece in to help the jaws grip better on the hammer, and the spacer piece split into pieces. One of the steel chips struck and lodged into employee's arms. The entry site bled but was wrapped up and the employee finished the job they were working on.
A bolt from the cheek plate in the primary jaw crusher dislodged and struck the employee in the mouth, causing a laceration to lip and broken teeth.
During a routine maintenance procedure on an air conditioning unit, site mechanic was injured when EE's left leg became entrapped by the equipment's bucket. Employee sustained a left ankle and foot fracture.
Employee was drilling a hole through the frame of a hydraulic press to install a guard when the drill caught and spun causing the employees hand to spin and EE's index finger struck the metal bracket causing the laceration.
Injured Employee was utilizing a torch to cut off two track bolts on an Excavator. As the employee finished the last bolt there was a spark causing them to pull the torch back. The torch then made contact with the right hand and burnt through the glove and burning the backside of their right hand causing a 2nd Degree Burn.
Employee was performing fire watch when employee received welding flash to eyes.
The employee was utilizing a torque multiplier tool to remove a tie rod form a hydraulic rock breaker and while releasing tension on the multiplier the ratchet that EE was using broke a tooth, skipped and quickly spun around and caught the employee's hand and caused a laceration.
Employee was using a drill to tighten the nuts on the lower side of the apron of a crusher when the load moved sideways to the ground catching the employee's finger causing a laceration and fracture.
1st EE & 2nd EE were welding/grinding single sand screw to mend it. 1st EE was under sand screw to weld when 2 EE began grinding causing sparks & debris to fly towards 1st EE. 1st EE removed self until 2nd EE finished. 1st EE was in full PPE; face shield safety glasses gloves hard hat & fall protection. EE reported it on 2/19. Diagnosed with conjunctivitis & prescribed eye drops.
Employee was holding a pipe with left hand and a 4 1/2 angle grinder with right hand when the grinder kicked back striking the employee's left thumb.
EE was using a Metabo with a 6" cutting wheel to cut a piece of 2" flat bar. While cutting the flat bar the Metabo kicked back off of the flat bar and the wheel struck EE in the leg just above EE's knee. EE was taken to the emergency room and received 5 staples. EE was released to return to work and will return to work the next work day.
IW was grinding on W13 drip pan on 8/14/23. At some point between 9:00AM and 12:00PM they felt something in their right eye. They washed it out on the night of 8/14/23. Employee did not seek medical treatment until 8/16/2023.
Employee was under a F-250 pick-up finishing up tightening the tab bolts for the cross-member for the transmission. While in the process, one of the tab bolts spun loose and caught the EEs palm of EE's hand.
Employee was operating a crane when it tipped to its side. The operator jumped out of the crane landing face first striking their forehead causing laceration needing stitches.
Employee was cutting a piece of wear metal out of a chute. As employee was cutting, the 42-inch x 21-inch x 1-inch plate popped free and smashed their left thumb between the chute and the plate.
While drilling out a cable fitting the drill bit pierced the employees glove and the outer portion of their left thumb. The employee received stitches.
Employee was climbing out of a chute with a lit torch in left hand when they swiped their right hand on the torch. Employee sustained a third degree burn to the right hand.
While torqueing wheel fasteners on a 785 truck the technician placed hand in the path of the reaction arm causing the hand to be crushed between the arm and the wheel.
OTR technician was repairing chains on a Loader. EE was cutting off a chain link and the cutting wheel shattered and a piece of the wheel struck the technician on the shoulder causing a laceration.
Employee was cleaning beneath the grizzly, at the dump hopper. When a clumped mass of material fell through the gap in the plate steel shielding the top grizzly, dropped approximately 15 feet, striking the employee on the top rear of their hardhat. Employee sustained a concussion without loss of consciousness and a neck sprain.
Employee was holding a piece of rubber with one hand and running a rigid brand multi tool with the other hand when the tool slipped striking the employee's index finger causing laceration.
Prussure Washer Hose Hung On Dozer Track
EE was attempting to unplug chute with a high pressure water hose. As soon as the EE opened the valve, the pressure caused them to lose their footing, falling backwards against the hand rail, and striking right ribcage on the top rail. EE sought medical attention on 8/30.
Employee was performing a maintenance task that resulted in an eye irritation injury.
As technician was torqueing wheel fasteners on the RRO of a 777 haul truck, the torque wrench shifted causing the wrench to crush the technicians left hand between the wrench and the wheel hub.
Employee was repairing the weatherhead on CR4 motor and while removing the bolts with a drill the adapter caught in the bolt and twisted the drill quickly forcing the employee's hand into a piece of expanded metal motor shaft guarding causing a laceration on employee's hand.
EE wrapped nylon strap around 8' pipe while excavator operator waited for signal to proceed. Operator bumped the joystick causing the bucket to move. EE attempted to move away, but EE's feet were submerged in sand, causing what was believed to be a sprained L ankle. EE requested medical attention on 4/5 due to continued pain while walking. X-rays revealed a fracture to EE's ankle.
Employee was in the process of working with a crane operator to remove a motor and motor base from the screening tower using a nylon lifting strap. When the lift began, the employee noticed the boom angle was too high and signaled the operator to boom down. The operator boomed up abruptly and snapped the nylon strap. The load dropped down and shifted, striking employee.
Employee was working on the jaw crusher. When EE cut the bolt, it ricocheted hitting the employee's face. Employee was treated at the emergency room and diagnosed with a fractured nose and fractured eye socket.
While training another person, employee looked back at the drill rig, the operator motioned the drill pipe. Employee noticed the drill pipe fell out of gripper arm. When employee touched the pipe to assist it back into the gripper arm. The pipe dropped and pinched the tips of fingers 3 & 4 on right hand
While spinning in a roof bolt, employee felt something get in right eye.
An employee had been doing night shift maintenance (welding/grinding). When they got home that evening, they felt an irritation and noticed a very small piece of metal in their eye. Employee was seen by a medical professional who removed the piece of metal. Employee was released to full duty, but it is unclear when the metal got in the eye.
An employee was helping another mag drill holes. The plate was cleaned and drill clear, another employee turned it on at the same time IE reached back in to move a rag. It became entangled in the bit and around IE's glove. This caused lacerations to ring finger and lacerations/fractures to the tip of middle finger, resulting in the loss of 1/2 inch of the tip of middle finger.
