EE WAS CLIMBING STAIR SCAFFOLDING TO THE 108 FT LEVEL TO FLY EQUIPMENT INTO PLACE WHEN HE FELL FROM THE STEAL STRUCTURE TO THE 45 FT LEVEL FLOOR BLEOW WAS WEARING BEAR HANESS AND DOUBLE LANYAR D WHEN FATALITY OCCURRED. MULTIPLE TIE OFF POINTS WERE PROVIDED.
