THE VICTIM HAD ALREADY TAKEN 1 CUT OUT OF #5 ENTRY AND IT WAS THEN BOLTED. HE AGAIN RE-ENTERED THE FACE AND HAD MINED THE LEFT SIDE OF THE ENTRY. The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.
