THE VICTIM HAD ALREADY TAKEN 1 CUT OUT OF #5 ENTRY AND IT WAS THEN BOLTED. HE AGAIN RE-ENTERED THE FACE AND HAD MINED THE LEFT SIDE OF THE ENTRY. HE WAS MINING THE RIGHT SIDE AND HAD TRAVELE D APPROXIMATELY 11 FEET BEYOND THE LAST ROW OF ROOF BOLTS, WHEN AN AREA OF ROOF (ABOUT 8 INCHES THICK, 11 FEET WIDE AND 18 FEET LONG) FELL ONT EH MIENR CRUSHING THE VICTIM IN THE DECK OF THE M
