Security Guard fatality
AT APPROXIMATELY 2:00 PM THE EVENING SHIFT MAN FOUND THE OUTSIDE OFFICE WAS BURNT TO THE GROUND.EMP'S BODY WAS FOUND IN RUBBLE. BECAUSE OF THE BODY'S POSITION AND A HISTORY OF CARDIAC PROBLEMS AS WELL LACK OF MEDICATION FOR THE 3 DAYS PRIOR AS REPORTED BY DAUGHTER, THE QUESTION WAS RAISED AS TO THE POSSIBILITY THAT DEATH WAS CAUSED BY OTHER FACTORS THAN THE FIRE. 988 Loader caught fire, was unable to be extinguished by site personnel, 911 was called. Two fire crews arrived on site, one fireman approached the loader by the rear tire. The plant manager was yelling at them to get away from the tire. It exploded killing the fireman by the tire.

The IE was hauling overburden in a Cat 793 end dump to a fill area. The front area on the end dump caught fire. The IE parked the truck on the fill and exited the end dump.
