VICTIM WAS PUTTING A TIRE AND WHEEL ASSEMBLY ONTO A WABCO 333 FT. SCRAPER. HE WAS USING THE COMPANY SERVICE TRUCK (CRANE) TO HOIST THE TIRE AND WHEEL. A CHAIN WAS USED TO CONNECT THE WHEEL ASS EMBLY TO THE CRANE. THE CHAIN SLIPPED OFF THE WHEEL CAUSING IT TO FALL ONTO THE VICTIM.
