EMPLOYEE WAS ATTEMPTING TO INSTALL A RIB PIN ON THE RETURN SIDE OF THE #1 ENTRY,IT IS BELIEVED THAT HE REACHED IN AND GRABBED THE DRILL STEEL WHILE AT THE SAME TIME ACTIVATING THE FAST FEED OF THE DRILL HEAD WHICH MOVED FORWARD AND CRUSHED HIM.
