THE EMPLOYEE WAS OPERATING A MOTOR GRADER IN REVERSE AND HIT THE BERM AND ROLLED PINNING THE VICTIM UNDERNEATH. THE VICTIM WAS NOT WEARING A SAFETY BELT.
