EMPLOYEE BACKED OVER THE BERM ON THE DUMP. THE TRUCK TURNED OVER, LANDING IN A MUD POND UPSIDE DOWN. THE MUD FORCED THE WINDSHIEDL INTO THE CAB. THE MUD SUFFOCATED THE INDIVIDUAL.
