EMPLOYEE WAS OPERATING ROOF BOLTER IN FACE ON #3 ENTRY ON THE 001 SECTION WHEN THE ATRS SYSTEM ON THE ROOF BOLTER ROLLED DOWN CATCHING HIM AGAINST FRAME OF ROOF BOLTER RESULTING IN DEATH.
