EE FAILED TO DE-ENERGIZE AUTOMATIC SAMPLER ARM AS INSTRUCTED BY MGMT. THE SAMPLER ARM CYCLED AUTOMATICALLY ACROSS THE INNER PORTION OF THE LOADOUT CHUTE EVERY 5 MIN. THE EVIDENCE SUGGEST THAT THE EE MAY HAVE CLIMBED INTO THE LOADOUT CHUTE, PLACED HIMSELF IN THE PATH OF THE SAMPLER ARM, AND WAS CRUSHED BY THE SAMPLER ARM. THE CO.
