EMPLOYEES WERE REMOVING 6 TON COUNTERWEIGHT FROM CRANE. EMPLOYEE REMOVED COUNTERWEIGHT PINS BEFORE CABLE WAS TIGHT. COUNTERWEIGHT BECAME UNSTABLE STRIKING HIM, CAUSING FATAL INJURIES.
